Requesting Records

Our Disclosure Services teams respond to requests for Access to Records by individuals who received services from the Society, as well as requests from others (“third parties”) for Disclosure of information.

Effective January 1, 2020, the Child, Youth and Family Services Act, 2017 (the CYFSA), gives individuals who received service from the children’s aid society the right to ask to see and get a copy of their personal information found in the records of service to them which are held by the CAS.

You can make a request for access to your personal information in those records by using our online Records Request System available here. Please select the request type applicable to your request and complete the information, including attachments where required.

If you have chosen to receive the records electronically, once we have prepared a response to your request you will receive an e-mail with a link to our secure portal, where you can download the response.

More information for parents:

  • A parent with custody of a child under the age of 16 may request personal information of their child, subject to some exceptions. If you are a person with custodial rights/decision-making and wish to request personal information of your child(ren) who are under the age of 16, please provide the names and dates of birth of your child(ren) and confirm that you are a custodial parent/guardian.
  • In some circumstances, we may ask you to provide documents confirming your authority to make the request in place of your child.

More information for individuals who were in CAS care:  

  • If you are interested in having confirmation of your former status with the CAS (e.g., Crown Ward/Extended Society Care, Society Ward/Interim Society Care), please complete our online application form, found here, selecting the request type “access to my records.” In the “reason for request” section, kindly indicate that you require a letter confirming your former CAS status. Alternatively, a PDF version of our “Request access to my records” form is available here for printing and completing offline.
  • If you require the completion of OSAP’s Verification of Status with Ontario's Children's Aid Society (CAS) form, please complete our online application form, found here, selecting the request type “access to my records.” In the “reason for request” section, kindly indicate that you require the completion of the Verification of Status form. Be sure to upload your signed and dated copy of the form. This must be the personalized version of the form downloaded from your OSAP account. The correct form will feature your name, address, and a unique bar code. Please do not submit a generic form, as it will not be accepted. Alternatively, a PDF version of our “Request access to my records” form is available here for printing and completing offline.

How to Request Disclosure of Records

Under the CYFSA, the Society is permitted to disclose personal information collected for providing services in certain circumstances. 

To make a request for Disclosure, please use our online Records Request System available here.

Please ensure you complete the appropriate form and attach the required documentation (e.g., consent of the individuals whose personal information you are requesting, court order, other authority).
